SCL Emergency Form

This form is utilized when there is a situation that may be a reason for someone to bypass the regular waiting list due to an extenuating emergency situation existing around the care of someone diagnosed with mental retardation and/or developmental disabilities (MR/DD).

An example of an emergency situation that may qualify would be the death of the primary caregiver of a person with MR/DD with no other caregiver available on a long-term basis. This is also a form that our staff can assist a person filling them out at any of our office locations. Please just contact the office location nearest you and ask for assistance from the Director or a Case Manager.
