This form is an application that is reviewed by the Division of Mental Retardation in Frankfort, Kentucky to determine eligibility to be placed on the waiting list for Supports for Community Living services. The waiting list is currently well over 2000 people for these waiver services. A person can download the application and fill out what he or she is comfortable filling out on the application and contact the office closest to you for assistance from a Case Manager in the completion and submission of the application.
